  • Application(s)for Boca Lakes: Below you will find the application for sales, new leases, and lease renewals. All applications can be emailed to, Fees must be turned in with application to be considered. All application fees are non-refundable. Please be advised BOCA LAKES HAS A 30 DAY APPLICATION PROCESS TIME FRAME. For more application information please refer to the "Application Information" page. 
  • Automatic Funds Transfer Form: If you would like to benefit from Automatic Funds Transfer, you can fill out the Automatic Funds Transfer form below. If you need assistance in filling our the form, please call the main office and speak to Giovanni who will assist you in setting this up (954) 426-0151. You can email the completed AFT form to Giovanni at 
  • Directory Information Form: This form gives the office the right to keep your email information on file, along with including your email in announcements to the community. Please complete the form with the correct email you would like all community announcements to be sent to. 
  • Change of Information Form: Any residents currently looking to update their current contact information, please complete this form entirely and email to:, or drop off at the Boca Lakes onsite office.
  • Recycling Bin Info: This form explains the rules and regulations on how to properly recycle in Boca Lakes. Please open form below to see rules and regulations. 
  • How to process your own Certificate in Cybersure: This documents contains detailed instructions on how to process your own certificate in Cybersure. Please see documents below for instructions. 
  • Club House Rental Agreement: For those interested in renting out the club house for a special occasion, please fill out the Club House Rental Agreement below. Once completed, please turn in the rental agreement to the onsite office along with the allotted fees. 
  • Alteration Request Form: Residents that would like to make improvements to the outside of their home are more than welcome. Before they are able to begin however they must complete the Alteration Request Form listed below, and turn in to the onsite office.
